Transition Care Program - Home Based Case Manager

The Royal Melbourne Hospital

 

  • Fixed term role until 16 October 2026.
  • 64 hours per fortnight (4 days per week).
  • Grade 2 Allied Health or Registered Nurse Grade 4A.
  • Gain valuable experience in providing goal-oriented case management for patients transitioning from hospital to home.

 

The Royal Melbourne Hospital is offering the opportunity for a Registered Nurse or Allied Health clinician to join the multidisciplinary TCP Home-Based team as a Case Manager at the Royal Park Campus. The Transition Care Program (Home-Based) provides short-term care to optimise the functioning and independence of older adults following a hospital stay, using a goal-oriented, therapy-focused approach.

 

The Role:  

  • The TCP case manager will hold a caseload and provide short-term case management to older adults following their discharge from hospital.
  • Conduct assessments over the phone and in the client’s home to assess risk, collaboratively establish goals and monitor progress.
  • Liaise with service providers to implement short term services and therapy for clients in their own home.
  • The TCP Case Manager will assist clients in engaging with ongoing supports and services, and help them adjust to long-term care arrangements.

 

About you: 

  • Qualified Allied Health Professional or Nursing and eligible for professional registration.
  • Looking to expand your knowledge in community services both in Melbourne Health and the wider community.
  • Demonstrate sound clinical skills and knowledge and be able to adapt these skills to problem solve and address client’s needs.
  • Demonstrate an understanding of goal centred care.
  • Demonstrate a good understanding of community programs, both within RMH and external.
  • Utilise excellent verbal and written communication skills to communicate with patients, carers, other health professionals and services providers.
  • Use your previous knowledge and experience to problem solve and be solution focused.
  • Established competency with Epic (Electronic Medical Record) preferred.
