About the role
Join the Acute Adult Inpatient Service to deliver social work support to consumers and their networks. You will complete psychosocial assessments, lead discharge planning, engage families and contribute to weekend ward rounds. Access supervision, development and opportunities to mentor students and graduates in a supportive, lived experience–integrated team.
Key responsibilities

Provide psychosocial assessments, discharge planning and case work

Work with families, carers and supporters, including after hours

Contribute to weekend rounds and timely discharges

Collaborate within a multidisciplinary and lived experience team

Mentor students and graduates and liaise with community services
About you
-
Qualified Social Worker (AASW eligible) with 2+ years’ experience
-
Skilled in family engagement and complex discharge planning
-
Comfortable working weekends in a 7-day servic

Committed to supervision and ongoing professional growth
