- Your role is diverse – you could be caring for patients with a new diagnosis, transitioning care from a paediatric to an adult service, on life-prolonging dialysis, donating a kidney to a loved one, traveling from Tasmania for a kidney transplant, accompanying the team to a regional hospital clinic to assess for transplant suitability, providing end-of-life advice
- Working with a dedicated and experienced social work team within our department of nephrology
- The opportunity for continuity of care for patients across the continuum of inpatient and outpatient care
- Qualifications which are acceptable for membership with the Australian Association of Social Workers
- Demonstrated ability to work collaboratively with a large, multi-disciplinary team
- Excellent interpersonal and communication skills
- Skills in comprehensive assessment, discharge planning and counselling
- Possess an understanding of relevant legislation and practice framework relating to family violence
