Social Work Clinician Grade 3 – Department of Nephrology

About the role

The Royal Melbourne Hospital Department of Nephrology is one of the largest dialysis and transplant services in Australia, with a strong history of innovation, research and collaboration with regional and rural care services.

 

As a Senior Social Worker Grade 3, you will provide specialist psychosocial care across the Kidney Care Service, supporting patients and families throughout their renal care journey.

 

Working within a multidisciplinary team, you will provide advanced assessment, counselling, advocacy and care coordination for patients with complex needs across inpatient, outpatient and community settings.

 

You will contribute to the development of Social Work practice through clinical supervision, education, quality improvement and service innovation, while supporting excellence in patient-centred care across a leading renal service.

 

Key Accountabilities

  • Provide high-quality, patient-centred social work services to patients and families across nephrology inpatient and outpatient settings.
  • Undertake comprehensive psychosocial assessments and provide evidence-based interventions to support patients through complex health journeys, including diagnosis, treatment, transplantation and end-of-life care.
  • Collaborate with multidisciplinary teams to support holistic care planning, informed decision-making and positive patient outcomes.
  • Provide psychosocial support, advocacy and counselling to patients, families and carers navigating complex medical conditions and life transitions.
  • Contribute to continuity of care by supporting patients across the continuum of care, including inpatient, outpatient and community-based services.
  • Participate in service development, quality improvement activities and professional development within the nephrology social work team.

 

About You

 

  • Qualifications which are acceptable for membership with the Australian Association of Social Workers
  • Demonstrated ability to work collaboratively with a large, multi-disciplinary team
  • Excellent interpersonal and communication skills
  • Skills in comprehensive assessment, discharge planning and counselling
  • Possess an understanding of relevant legislation and practice framework relating to family violence
