Progression of Care Navigator – Mental Health Services (Allied Health Grade 4)

About RMH Mental Health Services
The Royal Melbourne Hospital Mental Health Services provides specialist recovery-focused care for adults and older adults experiencing serious mental illness through trauma informed, evidence based and person-centred inpatient and community services.
About the Role
The Progression of Care Navigator supports timely discharge planning and consumer flow across the Adult Acute Inpatient Unit. Working closely with multidisciplinary teams and community providers, you will coordinate safe transitions of care, identify barriers to discharge and support improved consumer outcomes and continuity of care.
About You
You are an experienced mental health Social Worker with excellent coordination and stakeholder engagement skills and a strong commitment to recovery-oriented practice.
You bring:
- An approved degree in Social Work and eligibility for AASW membership
- An Approved degre in Occupational Therapy and registartion with Occupational Therapy Board of Australia
- Minimum 7 years’ experience in mental health assessment and treatment
- Strong understanding of discharge planning and community supports
- Excellent communication, organisation and relationship management skills
- Experience working collaboratively with consumers, carers and multidisciplinary teams
- Commitment to person-centred and recovery-focused care
- Strong motivation, highly effective communication skills and passionate abotu leading others.
