Patient Flow & Access Coordinator
The Royal Melbourne Hospital

 

  • Triage and assessment of referrals to the CLRAAC service including external referrals to and from RMH.
  • Permanent Part Time .6EFT
  • Based at City Campus, Parkville

Your new role:

  • Triage and assessment of referrals to the CLRAAC service, external referrals to and from RMH services
  • Management and maintenance of the patients on the waiting list for RMH Home First, Ambulatory & Complex Care admitted services, including outsourced services
  • Be part of an experienced multidisciplinary team  committed to the delivery of high quality patient centred care

About You:

  • We are seeking either an experienced nurse or allied health professional with demonstrated abilities to triage and coordinate internal, external and community referrals for Geriatric Evaluation Management (GEM) & Rehabilitation
  • High level clinical reasoning and problem solving abilities and highly developed critical thinking skills
  • Excellent communication skills and ability to liaise with multidisciplinary teams
