Palliative Care Community Health Nurse

The Victorian Palliative Care Advice Service (PCAS) is seeking a passionate and experienced Specialist Palliative Care Nurse to join our dedicated team. This remote role provides a unique opportunity to support Victorians living with life-limiting illness and their caregivers through expert, phone-based clinical advice, navigation, and support.

The Victorian Palliative Care Advice Service (PCAS) is a free and confidential, phone-based service providing expert advice, support, and guidance to all Victorians living with or caring for someone with a life-limiting illness or requiring palliative or end-of-life care.

Led by specialist nurses and supported by consultants from the Parkville Integrated Palliative Care Service, PCAS delivers timely, high-quality support to clinicians and the wider Victorian community, with a particular focus on improving access to care in regional and rural areas.

As a Specialist Nurse, you will serve as the primary point of contact—providing expert clinical advice, navigating callers to appropriate services, and helping manage the complexities of palliative care. This role demands exceptional clinical expertise, deep knowledge of the Victorian healthcare system, and excellent communication and interpersonal skills.

As a vital member of the team, you will operate within a nurse-led model supported by the Parkville Integrated Palliative Care Service, contributing to improved access to palliative care—especially in regional and rural communities.

Key Responsibilities:

  • Provide clinical advice and guidance within defined scope of practice, seeking assistance when appropriate.
  • Communicate complex medical information in plain language to patients, families, and carers.
  • Demonstrate excellence and autonomy in clinical decision-making relevant to the advice service.
  • Remain calm, collaborative, and focused in high-pressure or complex clinical scenarios.
