Play a vital role in implementing and evaluating an innovative model of care for live kidney donors at The Royal Melbourne Hospital (RMH). RMH has the largest kidney transplant program in Victoria and the second largest in Australia. Since performing its first kidney transplant in 1963, RMH has completed more than 3,400 kidney transplants, including achieving a milestone of 1,000 living donor transplants in 2019.
This unique opportunity allows you to work within a dedicated multi-disciplinary team to coordinate the assessment and care of patients referred to our new Living Donor Care Centre.
Your new role:
As a Kidney Transplant Coordinator (Living Donor), you will:
- Act as a resource for patients referred to the Living Donor Care Centre from primary care, regional hubs, and interstate.
- Coordinate investigations and preparation for potential live kidney donors at the John Perrett Kidney Centre, Elizabeth Street Campus.
- Partner with medical, surgical, and allied health colleagues to support potential live donors through every stage of their journey.
- Foster a cohesive and well-coordinated care pathway to ensure donors and recipients experience person-centred, timely and compassionate care.
- Participate in national data collection and quality improvement activities to advance best practice in living donor transplantation.
About you:
- AHPRA-registered Registered Nurse with post-graduate qualifications in nephrology nursing.
- Minimum 5 years nursing experience, including at least 2 years caring for kidney donors and recipients.
- Advanced assessment, problem solving, and organisational skills.
- Demonstrated commitment to high quality, evidence-based, patient-centred care.
- Excellent interpersonal and communication skills, with the ability to work collaboratively across multi-disciplinary teams.
