About The Royal Melbourne Hospital 

The Royal Melbourne Hospital (RMH) part of the Parkville Precinct and is one of Australia's leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs. 

 

We have a 0.7 EFT fixed term part time (April 2026) role available within the Hospital Admission Risk Program (HARP). This role aims to deliver best practice patient care when managing the HARP triage and referral workflows; engaging with clinical units and clients to assess and plan management and addressing risk factors which most likely to lead to hospital re-admission. This contributes to the overall HARP aim of improving clients’ health outcomes and reduce the risk of unnecessary representations. This position is based at the City Campus.

Your new role:

  • Providing a responsive and informed first point of contact for referrers to facilitate referrals to the HARP service
  • Triaging recently discharged clients who are at high risk of re-presenting to hospital, engaging with clinical units and clients to develop appropriate plans of care to address their health, psycho-social issues.
  • Actively support clients to make informed decisions about their treatment options and ongoing care needs, including appropriate medical follow up as required.
  • Provide support and advice on the management of complex clients to MH clinicians, HARP staff and community services
  • Provide a virtual care co-ordination service to clients following discharge from hospital.
  • Provide phone-based assessments re-enforcing discharge summary recommendations, clarifying medication changes, clarifying outpatient appointments, general assessments of supports available (formal/social/environmental/etc) and relevant on-referrals to assist the client.
  • Ensure clients receive adequate health education / information in an appropriate and accessible format to support their post discharge care needs.

About you:

  • Qualified Registered Nurse with valid AHPRA registeration
  • Experience in complex needs assessment, care planning and coordination for clients with chronic disease and complex psychosocial needs
  • Understand factors contributing to hospital admission in clients with complex needs
  • High level knowledge of Community Services
  • Demonstrated ability to be flexible, energetic, innovative, and work cooperatively in a multidisciplinary team as well as autonomously
  • Competent IT user, with well-developed written and verbal communication skills
