About the Role

 

We are seeking a motivated Allied Health Assistant to join our HARP team within Home First, Ambulatory & Complex Care. 

 

You will support multidisciplinary clinicians to deliver person-centred care to people with chronic and complex conditions, helping to improve health outcomes and reduce avoidable hospital presentations.

 

Key Accountabilities

 

  • Assist with exercise programs and functional interventions
  • Support equipment provision and client education
  • Assist clients to engage with appointments and self-management
  • Provide administrative support to care coordination
  • Work across home, community, and outpatient settings

 

About You

 

You will be a flexible, team-focused Allied Health Assistant who brings strong communication skills and a commitment to high-quality care. Together with:

 

  • Certificate IV in Allied Health Assistance (or equivalent)
  • Strong interpersonal and teamwork skills
  • Ability to work in a multidisciplinary environment
  • Competence with IT systems
  • Current driver’s licence; Working with Children Check, Police Check & immunisation compliance
  • Commitment to the Melbourne Way (People First, Lead with Kindness, Excellence Together)

 

Desirable

  • Experience in community or home-based care
  • Experience working with chronic disease, older people or complex needs
