About the Royal Melbourne Hospital (RMH):
RMH part of the Parkville Precinct and is one of Australia's leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs.
About the role:
- Support safe and timely discharge for patients with disability to return to the community.
- Provide excellent evidence based and patient centred care including discharge planning, care coordination, and client advocacy
- Utilise specialised skills and knowledge in relation to disability, communicating between patients and caregivers, and supporting ongoing community needs
- Demonstrate excellent communication strategies with patients, relatives, staff and relevant services to provide appropriate services, information and leadership
- Actively lead and participate in Allied Health quality improvement initiatives and research
About You:
- You have demonstrated health industry experience with a collaborative leadership style
- You have excellent communication skills and a passion for continued professional development for yourself and others.
- You are a Qualified health professional with current AHPRA registration (where relevant), or eligible for membership of relevant professional association
- You value working with supportive and dedicated health professionals and multidisciplinary teams.
