About the Royal Melbourne Hospital (RMH): 

RMH part of the Parkville Precinct and is one of Australia's leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs.

About the role:

• Support safe and timely discharge for patients with disability to return to the community.
• Provide excellent evidence based and patient centred care including discharge planning, care coordination, and client advocacy
• Utilise specialised skills and knowledge in relation to disability, communicating between patients and caregivers, and supporting ongoing community needs
• Demonstrate excellent communication strategies with patients, relatives, staff and relevant services to provide appropriate services, information and leadership

• Actively lead and participate in Allied Health quality improvement initiatives and research  

About You:

• You have demonstrated health industry experience with a collaborative leadership style
• You have excellent communication skills and a passion for continued professional development for yourself and others.
• You are a Qualified health professional with current AHPRA registration (where relevant), or eligible for membership of relevant professional association
• You value working with supportive and dedicated health professionals and multidisciplinary teams.
