The Continuing Care Team (CCT) Coordinator plays a key leadership role in overseeing the efficient operation of clinical services across the Northwest and Inner West regions. Leading a dynamic team, the coordinator ensures the delivery of high-quality, evidence-based care, while managing resources and driving service integration. The role also involves strategic planning, policy development, and fostering strong community and clinical partnerships, with a focus on improving accessibility and outcomes for youth and families.

About the role:
- Provide strategic leadership and direction for the development and delivery of clinical services in CCT-Region A
- Oversee resource management, ensuring effective use of human, financial, and technological resources
- Lead quality care initiatives, monitor clinical processes, and manage a limited clinical caseload
- Foster a supportive and collaborative work environment, offering supervision and professional development to staff
- Build strong partnerships with key community agencies, contribute to research, and support service innovation
About you:
Formal Qualifications
- This is a multidisciplinary role open to Registered Psychiatric Nurses, Social Workers, Psychologists or Occupational Therapists (must hold relevant degree, registration and/or membership as per attached Position Description)
Essential
- Over 10 years of experience in clinical mental health practice, specifically with young people experiencing mental health disorders.
- Proven ability to lead and manage multidisciplinary teams, ensuring high standards through supervision, training, and professional development.
- Strong case management skills, with a focus on supporting young people and their families/carers
- Expertise in building effective relationships and collaborations with community services and stakeholders.
- In-depth knowledge of mental health legislation, early intervention strategies, and a commitment to ongoing professional development.
