Clinical Nurse Consultant – Palliative Care RAPID Service

  • Part Time – Fixed Term (Four-Month Secondment)

The Royal Melbourne Hospital is one of Victoria's leading public health services, renowned for excellence, innovation and compassionate care. An exciting opportunity is available for a suitably qualified Registered Nurse to undertake a four-month secondment as Clinical Nurse Consultant within our Palliative Care RAPID Assist service.

This is a unique chance to work alongside a dedicated team of healthcare professionals delivering rapid palliative care intervention for patients known to the Parkville Integrated Palliative Care Service. RAPID Assist is a hospital-to-home outreach service that supports patients to continue receiving expert, high-quality palliative care in their own home or Residential Aged Care Facility — reducing unplanned hospitalisations and improving outcomes for patients and their families.

This role is fully supported by senior and experienced clinicians, with a genuine focus on expanding and developing your nursing capabilities and confidence in this specialised area.

We are seeking applications from registered nurses who demonstrate a strong interest and with experience in Palliative Care.

Your new role

  • Deliver timely, expert palliative care assessment and intervention across community and RACF settings
  • Act as a specialist clinical resource for the Parkville Integrated Palliative Care Service
  • Coordinate care with community palliative care services, GPs, Hospital in the Home, and Residential In Reach teams
  • Support patients and families through goal-setting, education and care planning
  • Collaborate with the multidisciplinary team to optimise patient outcomes and care transitions
  • Contribute to quality improvement activities and service innovation
  • Monitor and support achievement of RAPID Assist KPIs

About you

  • Registration with AHPRA as a Registered Nurse
  • Keen interest and demonstrated experience in Palliative Care
  • Advanced clinical assessment and critical thinking skills
  • Excellent communication, interpersonal and consumer engagement skills
  • Ability to work collaboratively within a multidisciplinary team and autonomously in a senior role

Desirable: 

  • Community Nursing experience
  • Leadership level experience in Palliative Care
  • Experience in Quality Improvement activities
  • Postgraduate study in Palliative Care
