Be part of a progressive Melbourne Health - Aged Care Assessment team, based at The Royal Melbourne Hospital, Royal Park Campus.
Part time and Full time negotiable. Fixed term to 30 June 2026.

Your new role:
- Conduct thorough assessments of older individuals to determine their care needs, preferences, and eligibility for aged care services.
- Work with clients, families, and care providers to develop personalized care plans that support the individual's physical, emotional, and social needs.
- Ensure that all assessments are completed in accordance with current legislation, policies, and quality standards.
- Collaborate with other health professionals, government agencies, and service providers to coordinate services and ensure holistic care.
- Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process.
- Monitor and review care plans to ensure they remain relevant and effective in meeting the changing needs of clients.
About you:
- Experience or qualifications in Aged Care, Nursing, Social Work, or a related field as per supportive documents.
- Appropriate qualifications
- Proven clinical experience
- Strong communication, empathy, and assessment skills.
- Knowledge of aged care services, policies, and regulations.
- A commitment to delivering person-centered care and advocating for older adults' rights.
- Ability to work independently and as part of a multidisciplinary team.
- Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite)
- Previous experience performing in a KPI environment to ensure good quality client outcomes
