Clinical Assessor

The Royal Melbourne Hospital

·         Be part of a progressive RMH - Aged Care Assessment team

·         Based at The Royal Melbourne Hospital, Royal Park campus

·         Fixed term to 30th June 2026

 

The Royal Melbourne Hospital’s Assessment Service is a component of the My Aged Care Assessment workforce. We are seeking Clinical Aged Care Needs Assessors who can undertake assessments of older people requiring access to Commonwealth funded programs.

The Clinical Assessors sits alongside Non-Clinical Assessors and provide clinical support to the Non-Clinical Assessors.

 

Your new role:

  • Conduct thorough assessments of older individuals to determine their care needs, preferences, and eligibility for aged care services.
  • Work with clients, families, and care providers to develop personalized care plans that support the individual's physical, emotional, and social needs.
  • Ensure that all assessments are completed in accordance with current legislation, policies, and quality standards.
  • Collaborate with other health professionals, government agencies, and service providers to coordinate services and ensure holistic care.
  • Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process.
  • Monitor and review care plans to ensure they remain relevant and effective in meeting the changing needs of clients.

About you:

  • Experience or qualifications in Aged Care, Nursing, Social Work, or a related field as per supportive documents.
  • Appropriate qualifications
  • Proven clinical experience
  • Strong communication, empathy, and assessment skills.
  • Knowledge of aged care services, policies, and regulations.
  • A commitment to delivering person-centered care and advocating for older adults' rights.
  • Ability to work independently and as part of a multidisciplinary team.
  • Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite) 
  • Previous experience performing in a KPI environment to ensure good quality client outcomes
