The Royal Melbourne Hospital (RMH) part of the Parkville Precinct and is one of Australia's leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs.
RMH are seeking a Clinical Nurse Consultant to join our Residential In Reach (RIR) team. Based within the Acute Aged Care Program, you will play a key role in providing timely, hospital-level care to residents in Residential Aged Care Facilities, helping to prevent unnecessary emergency department presentations. This autonomous and rewarding role involves triage, clinical assessment, and collaboration with GPs, Geriatricians, and community services to deliver person-centred, evidence-based care in a fast-paced, community-based healthcare setting.

About the role:
- Provide advanced nursing assessment and acute clinical care directly to residents in Residential Aged Care Facilities, reducing avoidable hospital presentations.
- Work autonomously and collaboratively with GPs, Geriatricians, and community services to deliver timely, person-centred care.
- Triage referrals, conduct face-to-face assessments, and support early intervention strategies within a dynamic, consultative nursing role.
- Maintain clear and timely communication with key stakeholders through telehealth, EMR, and written documentation to support integrated care planning.
- Liaise closely with residents and their families to ensure holistic, person-centred care that aligns with individual values, preferences, and goals of care.
About you:
Qualifications
- Registration as a Registered Nurse under the Australian Health Practitioner Regulation National Law Act (2009) with the Nursing and Midwifery Board of Australia.
Essential
- Current, unrestricted Victorian Driver’s License
- Post graduate qualifications in critical care
- Minimum 5 years’ experience post registration
- Highly developed assessment, clinical reasoning, problem solving and prioritisation skills
- Strong interpersonal and communication skills both written and verbal
- High level of reliability and professional conduct
- As of 1 January 2026 all new emplkoyees will require a valid Victorian employee Working With Children's Check
