Join the RMH Residential In Reach (RIR) team as a Clinical Nurse Consultant in our Post Hospital Support stream, supporting older adults as they transition from hospital back to residential aged care.

Job Description: 

CNC - Residential Inreach Post Hospital Support

 

  • 64 hours per fortnight | Fixed term until June 2028

This nurse-led service works across hospital and residential aged care settings to provide proactive discharge planning, intensive post-discharge clinical support and coordinated care for up to four weeks. You’ll work closely with the CNC Coordinator, Geriatric Medicine team, residential aged care providers, residents and families to support safe, seamless and person-centred transitions.

You’ll have the opportunity to use your clinical expertise and autonomy to identify emerging needs, build confidence among aged care teams and families, and help prevent avoidable hospital readmissions.

Key Accountabilities

  • Identify and support patients requiring complex transition planning
  • Provide clinical assessment and support following discharge
  • Coordinate care across hospital and residential aged care settings
  • Support RACH staff and families to manage ongoing care needs
  • Identify and respond to emerging clinical and support needs

About you

You are an experienced Registered Nurse, with at least 5 years’ post-registration acute nursing experience, with experience in acute geriatric or community care highly regarded, together with:

  • Strong clinical assessment, reasoning and decision-making skills
  • Confidence working autonomously with complex and changing needs
  • Excellent communication and relationship-building skills across care settings
  • Strong clinical documentation and EPIC skills
  • The ability to coordinate care and build effective relationships with residents, families and healthcare providers
  • A flexible, reliable approach and the ability to prioritise effectively under pressure
  • An understanding of chronic disease management, community care and transitions from hospital to residential aged care
  • Postgraduate qualifications in critical care will also be valued.
