  • Part Time Fixed Term, 48hrs per fortnight (Parental Leave Cover)

We are looking for a passionate and dedicated individual to join our HARP Complex Care team based at our City Campus, working with patients in transition from hospital to community.

About the Role:

  • Provide optimal care to meet patients’ needs by conducting Medication Management Review of HARP clients, providing reports for treating physicians and GPs, and on-referrals to appropriate other services
  • Provide best practice patient care through developing and implementing care plans to address medication-related issues, through coordination of care with clients/carers, HARP clinicians, GPs and other services
  • Undertake a Care Co-ordinator role for clients with medication related complex care needs, including comprehensive assessment, care planning and service coordination to improve client health outcomes and prevent presentation or readmission to hospital
  • Provide support and advice on the management of complex clients to HARP staff and community services

 

About you:

  • Qualified Pharmacist, with relevant AHPRA registration, and minimum 18 months experience as a qualified pharmacist
  • Demonstrated ability to practice in accordance with Society of Hospital Pharmacists of Australia (SHPA) Guidelines
  • Current Victorian Driver’s License
  • An understanding of the transition of patients and their medication management from hospital to the home environment
  • Demonstrated ability to be flexible, energetic, innovative, and work cooperatively in a multidisciplinary team as well as autonomously
  • Competent IT user, with well-developed written and verbal communication skills
  • Home Medicine Review Accreditation
