Renal Transplant Coordinator

About the Role

This advanced nursing role is responsible for coordinating and supporting patients throughout their kidney transplant journey, including assessment, waitlisting, transplantation and post-transplant follow-up. Working closely with a multidisciplinary team, you will provide expert clinical coordination, patient education, care planning and support for transplant recipients, donors and their families.

The role also involves collaborating with referring health services across Victoria to improve access to transplant assessment and care.

Key Accountabilities

In this role, you will:

  • Coordinate kidney transplant assessments, investigations, education and clinical reviews for transplant candidates.
  • Support patients and families throughout the transplant pathway, providing expert advice and care coordination.
  • Coordinate post-transplant follow-up, monitoring and education for transplant recipients.
  • Collaborate with multidisciplinary teams and regional referring services to facilitate timely access to transplant care.

About You

Essential

  • Registered Nurse with significant experience in renal and/or transplant nursing.
  • Postgraduate qualification in Renal Nursing or equivalent.
  • Demonstrated ability to work effectively within multidisciplinary teams.

Desirable

  • Experience participating in clinical research or quality improvement activities.
  • Willingness to travel across regional Victoria and work flexibly to support transplant services.
