About the Role
Join one of Australia's leading renal services and play a key role in coordinating care for patients across Victoria's statewide dialysis network. Working closely with medical, nursing and allied health teams, you will support patients through their dialysis journey, facilitate access to home and regional services, and contribute to innovative models of care. This autonomous and rewarding role combines advanced clinical practice, care coordination, leadership and quality improvement to enhance patient outcomes and shape the future of renal care.
Key Accountabilities
In this role, you will:
- Coordinate care and service pathways for patients commencing and transitioning between dialysis services.
- Collaborate with multidisciplinary teams to support safe, timely and patient-centred care.
- Facilitate patient and carer education regarding dialysis management, medications and self-care strategies.
- Contribute to quality improvement, clinical governance, education and service development initiatives across the Kidney Care Service.
About You
- Extensive renal nursing experience, including dialysis care and patient coordination.
- Demonstrated ability to work autonomously and collaboratively within a multidisciplinary team.
- Strong communication, stakeholder engagement and care coordination skills.
Desirable
- Postgraduate qualifications in renal nursing or a related specialty.
- Experience in quality improvement, research and clinical governance activities.
