Clinical Nurse Consultant - Residential In-Reach
The Royal Melbourne Hospital - Royal Park Campus

  • Part time 0.5 EFT. 11 months fixed term
  • Applicants will also be considered for upcoming fixed-term positions with varying EFT
  • Royal Park Campus location, discounted onsite parking

About the role:

  • Provide advanced nursing assessment and acute clinical care directly to residents in Residential Aged Care Facilities, reducing avoidable hospital presentations.
  • Work autonomously and collaboratively with GPs, Geriatricians, and community services to deliver timely, person-centred care.
  • Triage referrals, conduct face-to-face assessments, and support early intervention strategies within a dynamic, consultative nursing role.
  • Maintain clear and timely communication with key stakeholders through telehealth, EMR, and written documentation to support integrated care planning. 

About you:

Essential

  • Post graduate qualifications in critical care
  • Minimum 5 years’ experience post registration
  • Highly developed assessment, clinical reasoning, problem solving and prioritisation skills
  • Strong interpersonal and communication skills both written and verbal
  • High level of reliability and professional conduct
