Colorectal Nurse Coordinator

  • Full-time Ongoing position, 80 hours per fortnight including ADO
  • Drive streamlined colorectal care via triage, telehealth, and waitlist oversight
  • Based in Parkville, with close transportation links

The Royal Melbourne Hospital (RMH) part of the Parkville Precinct and is one of Australia's leading public hospitals, providing a comprehensive range of specialist medical, surgical, and mental health services, as well as rehabilitation, aged care, specialist clinics and community programs.

RMH are seeking a Colorectal Nurse Coordinator to join their dynamic team at the City Campus. In this pivotal role, you will collaborate with medical staff, nurses, and allied health professionals to streamline and enhance the care of patients with colorectal conditions. Focusing on triage, telehealth integration, and waitlist management, the coordinator ensures timely, effective, and patient-centred care throughout the entire colorectal care pathway.

About the role:

  • Coordinate patient care by working closely with multidisciplinary teams to ensure timely, effective, and patient-centred treatment for individuals with colorectal conditions
  • Manage triage processes for new referrals, optimizing waitlist management and streamlining access to specialist services
  • Integrate digital health strategies such as telehealth and Health Hub to enhance patient engagement and care delivery
  • Collaborate with GPs and external services to improve referral pathways, ensuring seamless communication and continuity of care
  • Lead quality improvement initiatives to enhance service delivery, patient outcomes, and operational efficiency within the colorectal care pathway

 

About you

Formal Qualifications

  • Registered Nurse with current AHPRA status and relevant nursing qualifications

 

Essential

  • A minimum of 5 years of acute nursing experience
  • Advanced assessment, clinical reasoning, and problem-solving skills, with experience in the management of colorectal patients
  • Excellent interpersonal and communication skills, with strong self-motivation and the ability to effectively prioritize and collaborate within a multi-disciplinary team
  • Proven ability to be innovative, resourceful, and adaptable to change

 

Desirable

  • Experience in colorectal surgical nursing
  • Experience in clinically based nursing education
  • Experience in management, quality or clinical education
  • Postgraduate qualifications in a relevant area of specialty
