About the service:
Our Home First Ambulatory and Complex Care service is seeking an Associate Nurse Unit Manager to join our Geriatric Evaluation and Management subacute care team on ward Aged Care 2, which is a 20-bed ward. We deliver physical and cognitive rehabilitation care for patients with chronic or complex conditions that are associated with ageing, cognitive dysfunction, or disability. We have a diverse patient cohort providing services to adults from 65 to over 100 years of age!
Our team are dedicated to support the next stage in the patient journey to full recovery after an injury, illness or surgery. We ensure that our patients gain the most independence and functionality that is possible with the support of our skilled multidisciplinary team.
We are passionate about supporting our team to deliver the best care and support their growth and development via education opportunities, team nursing models and preceptorship.
Your new role:
- Ensure there is provision of high-quality standards of patient care. This includes the assessment, planning, implementation, and evaluation for care in collaboration with multidisciplinary team to achieve the desired patient outcomes.
- Assume the responsibilities of the manager maintaining professional and operational standard out of hours.
- Ensure individual patient plans, estimated discharge dates, progression of care, next destination are updated on the Patient Journey Board (Electronic Medical Records system) each shift and understands the importance of this workflow to facilitate effective and timely discharge planning and communicates effectively with bed management team and multidisciplinary teams.
- Provide clinical leadership and collaborate with the multidisciplinary team to address priority issues, in relation to providing care to consumers with complex care needs
- Provide support and assistance to the NUM in the operational leadership of the unit as required
- Contribute to the development of an effective discharge plan and provide appropriate patient education and referral to appropriate community providers
About you:
- Registered Nurses that are eligible to apply must have a minimum of 5 years acute or subacute experience with leadership experience working in the Nurse in Charge role and/ or have experience in managing a portfolio
- Demonstrated ability to manage a team
- Professional role model, demonstrating consultative, innovative and responsible leadership
- Demonstrated provision of high-quality patient care
- Highly developed and effective communication skills and are passionate about professional development
