The Opportunity:
The Continuing Care Team (CCT) Coordinator is a key leadership position within Parkville Youth Mental Health Service (PYMHWS), overseeing one of two large CCTs that provide care to young people aged 12–25 and their families/carers across the PYMHWS catchment. PYMHWS delivers early intervention and specialist mental health care through multidisciplinary teams across multiple sites, guided by evidence-based, culturally safe, and trauma-informed practices.
Our service is made up of compassionate, curious, and committed people. As we continue to grow, we are looking for a motivated CCT Coordinator to provide operational and clinical leadership, team supervision, and strategic oversight, ensuring high-quality, recovery-oriented care across the CCT. This role will lead service planning, support integrated care pathways, and contribute to the ongoing development of new sites and innovative models of youth mental health care.

About the Role:
The CCT Coordinator oversees the day-to-day operations of the Continuing Care Team, ensuring smooth service delivery, effective resource management, and adherence to clinical standards. The role includes supporting staff professional development, monitoring service quality, and implementing initiatives that improve accessibility and outcomes for young people.
This position works closely with clinical stream leads and program managers to facilitate integrated care across multiple sites, and contributes to planning, evaluation, and the introduction of new service models within the PYMHWS catchment.
What You'll Do:
- Lead the operational management of the Continuing Care Team, including workflow coordination, resource allocation, and implementation of service initiatives across multiple sites.
- Oversee staff supervision, performance development, and capability building, ensuring team members are supported to deliver high-quality, recovery-oriented care.
- Monitor and evaluate service quality and outcomes, contributing to continuous improvement, program development, and implementation of innovative care models.
- Support integrated care pathways and cross-stream collaboration, facilitating communication and coordination between clinical teams, internal programs, and external partners.
About You:
An experienced and motivated mental health leader, passionate about coordinating multidisciplinary teams and driving high-quality, recovery-focused care for young people and their families. You have a strong commitment to service innovation, integrated care pathways, and inclusive, culturally safe practices, ensuring that care meets the diverse needs of all young people across the PYMHWS catchment.
✔️Eligible disciplines: Registered Nurse (RPN Grade 5), Occupational Therapist, Social Worker, or Psychologist (Grade 4)
✔️ Relevant registration and qualifications in mental health, leadership, or related fields (see position description for details)
✔️ Minimum 10 years’ experience in youth mental health or tertiary mental health, with demonstrated ability to lead, supervise, and develop multidisciplinary clinical teams
✔️ Strong experience in operational and strategic leadership, including workforce planning, budget oversight, and service development across multiple sites
✔️ Motivated by youth-focused, recovery-oriented, and culturally safe care, with a commitment to service innovation and improving accessibility for diverse communities
